Default What is a vented barrel extension? Rule 6.2.3 page 30

Is it a brake or something under the handguards?

If you look on page 28 describing the M14/M1A, you will see the phrase, "Vented barrel extensions with the same external dimensions as the M14 flash suppressor, but without the bayonet lug, may be installed."

The reference on page 30 is also talking about flash suppressors ("The flash suppressor and bayonet lug may be removed or the rifle may be manufactured without a flash suppressor or bayonet lug. A vented barrel extension may be attached.") It seems to mean that a muzzle device that is not a muzzle brake can be attached in place of the flash suppressor for those benighted jurisdictions that prohibit flash suppressors.
