What's the correct way to submit a FOIA request

I have been reading multiple threads on submitting FOIA requests and I'm still not sure what is the correct way to do this. I've seen some have reference to using a form and selecting different departments, and others where you send an email. A pinned topic on this would be nice to have as a quick reference.

I sent a similar email and received my reply in about 2 weeks:

If you're doing the foia.gov, it's for the Dept. of the Army

Just submitted a request via FOIA.gov. Will try sending emails after I get a response back from this one.

There is no fee, but you have to offer to pay a fee. Even if just $1.

I got my initial request back from Mickelle within a week or so. Very easy. I have since submitted a request to the Navy but haven't heard anything in two months other than they have received it.
